Teams assist and support individuals dealing with a mental health crisis by going to the individual wherever they are in Salt Lake County. Accessed through the Crisis Line.

Phone line open 365 days a year from 9:00 AM to 10:00 PM. Provides a friendly and understanding person for those who need someone to talk to and enables individuals with a mental health challenge to talk to a peer about issues they are facing in their lives.

LEVEL OF CARE GUIDELINES

Outcome Questionnaire/Youth Outcome Questionnaire reporting (OQ/YOQ)

The OQ/YOQ is a tool designed to support treatment planning and a therapeutic focus for clients.  The OQ / YOQ measures are to be administered at intake and every 30 days during treatment. The State of Utah requires it’s use. Optum SLCo partners with Salt Lake County Division of Behavioral Health Services and OQ Measures to make the OQ/YOQ available to all providers at no cost.  If you have questions regarding access to the OQ/YOQ, please contact the Optum SLCo Compliance Manager at 1-877-370-8953.
